So, imagine you're in the gym. You're exercising on a machine, and in front of you is another row of machines with people exercising on them. The person in front of you is wearing black capris. Except, either they are not actual exericse tights or they are worn thin, because you learn that the person is wearing a thong, because you can see it through the fabric.
Do you say anything?
The trouble is, I was exercising, and I would have had to stop, go and interrupt the other person's exercising, and explain the issue of which they may not have been aware. I did not want to stop, so I did nothing. Also, what if the person knew, and didn't care? Then again, what if the person did care, in fact really did not want everyone to know their underwear preferences?

On balance, the greatest happiness (or least unhappiness, anyhow) all around would seem to be served by saying nothing.
Usually I'm not stuck in the same place, with nothing to do but contemplate the wardrobe spectacle.
